Applications are now open for local organizations to receive microgrants to host community-based wellness fairs that will expand access to health resources, services, and ongoing care for more than 22,800 Chicago residents.
Mayor Brandon Johnson and the Chicago Department of Public Health (CDPH) today announced the launch of the Community-Centered Wellness Microgrant Program, a $600,000 investment that will support local organizations in hosting community-based wellness fairs that connect Chicago residents with health services, resources, and ongoing care. CDPH also announced the selection of SGA Youth & Family Services as the Lead Coordinating Organization to administer the program.
“Good health starts with having access to the resources, services and support you need in your own community,” said Mayor Brandon Johnson. “This investment strengthens the community organizations that are closest to the people they serve, empowering them to bring health resources, education and support directly into neighborhoods. By investing in trusted local partners and community-led solutions, we are making it easier for residents to access the care they need and helping build a safer, healthier Chicago for all.”
SGA Youth & Family Services will be awarded $600,000 to operate the microgrant program. Chicago community-based organizations will be able to apply for funding to plan and deliver health and wellness events in neighborhoods across the city. The program will prioritize communities experiencing lower life expectancy and communities where residents are uninsured, underinsured or face barriers to accessing care.
By the end of the program period in November 2027, the initiative aims to measurably increase access to health resources and services for more than 22,800 Chicago residents.
“Every Chicagoan should have access to the care and support they need to live a thriving, healthy life,” said Dr. Garth Walker, CDPH Commissioner. “This investment will help local partners help us to achieve our goal of closing the life expectancy gap across Chicago by creating wellness activities that respond to resident needs, connect residents with essential services and strengthen community health and well-being.”
Through the Community-Centered Wellness Microgrant Program, participating organizations will host community-based wellness fairs that provide non-clinical health services, education and resources. Events may include blood pressure and cancer screenings; nutrition education and healthy cooking demonstrations; exercise and movement activities; and workshops on social, economic and other factors that affect health outcomes. The program is designed to create pathways that connect residents with ongoing support services.
SGA will manage the application and selection process, support participating organizations and coordinate program implementation in partnership with CDPH.

The Community-Centered Wellness Microgrant Program reflects CDPH’s continued investment in community-driven public health efforts. The program recognizes that health promotion is most effective when communities have a meaningful role in identifying local needs and determining how those needs are addressed.
The program will offer two funding cycles. The first funding cycle opened Monday, August 17, 2026, and applications will be accepted through 11:59 p.m. on Wednesday, September 9, 2026. Applications for the second funding cycle will open in January 2027.
A pre-proposal webinar will be held Tuesday, August 25, 2026, at 1 p.m. to provide prospective applicants with an overview of the Community-Centered Wellness Microgrant Program and application process.
